Thank you for scheduling your appointment at the Benjamin Eye Institute.
We appreciate your trust and are looking forward to working with you.
In these challenging times of the COVID-19 pandemic our highest priority is the safety of our patients and staff. While Virtual MD visits are still available we understand that there are times when a hands on visit is required.
To make sure your visit with us is safe, efficient and productive we have instituted some changes to the in-person patient encounters, to minimize the time you spend in the office without compromising the quality of care you receive. This process has been established based on CDC, HHS, American Academy of Ophthalmology recommendations.
We respectfully request adherence to the following guidelines:
- You must wear a mask or a face covering to enter the building.
- Your appointment time is strictly reserved for you. Since the waiting room is no longer available, we are unable to accommodate patients who come early or late for their appointment, so we ask you to arrive at our suite on time.
- Each patient is assigned an exam room and a technician in advance. We will not be able to see you if you are more than 5 minutes late to your appointment and will need to reschedule.
- Once you arrive at the building, please text or call us (310)275-5533 alerting us to your arrival. We must avoid crowding patients in the hallway.
- If you arrive early please wait in the car or outside the building until we ask you to come up via text.
- Once at the entrance to the suite, please ring the bell and you will be met with our staffer who will check your temperature with a non contact thermometer, ascertain presence of a face covering and escort you to a private exam room.
- Please note, all administrative functions MUST be completed prior to your arrival using a mobile device or desktop computer to check in and update demographic, insurance, payment and health information (see instructions below).
Now that you have an appointment scheduled you will need to create TWO portal accounts whether you’re a NEW or EXISTING patient.
STEP 1

Create a BREEZE account for administrative, scheduling and financial aspects of your care.
You should have received an email or text message with a link to create the Breeze account. If you have not received such an invitation, check your spam folder. If you don’t find it, please text or call (310) 275-5533 to request that link be resent.
Select the unique personalized link and follow the step by step instructions.
If you experience any difficulties with any of the steps in the process, please contact us for help.
STEP 2

Creating an Electronic Health Record (EHR) patient portal account is essential for the clinical aspects of your care.
You should have received a separate email/text with an invitation to set up the EHR portal on EMA.
If you have signed in before, but now don’t remember your password, please use the “Forgot Password” function to have the password instructions emailed to you.
If your portal has been activated in the past but you don’t recall your username, please call our office and we will help you with resetting your credentials.
Please note that the resets and invitations expire after 72 hours.
Once you register and log in to the EMA portal, please update your medical history as instructed.
