On the Use of Intelligent Machines to Achieve Precise Diagnoses

Ophthalmologist Arthur Benjamin is known not only as an eye surgeon at his namesake Benjamin Eye Institute, but  also teaches students and future doctors undergoing residency at the University of California, Los Angeles (UCLA).  It is for this reason that he stays current on all the newest innovations, which he strives to epitomize in practice.  We should, however, first talk about his theory of success.

Can you please articulate what constitutes good medical treatment?

Let’s start from the beginning.  The patient comes in.  First of all, you need to listen to them very carefully and attentively.  Take into account their medical history.  This includes everything about them, and not just their illnesses (diabetes, thyroidism), symptoms, or injuries, but also what a person does on a daily basis.  What do they use their vision for?  An artist or photographer uses it in one way, but a film director or musical conductor uses it in another.  Similarly, everyone drives a car and thus needs to see the road, but a professional driver requires additional considerations.  Even engineers will have their own particular spiel.  Consequently, an experienced physician’s assistant or doctor must know where to lead the conversation.  So, you may ask, what is it that I’m getting at?  The answer is that we need to develop detailed solutions in order to achieve the best results,  e.g. addressing the patients’ complaints (loss of visual acuity, discomfort, pain, etc.), and this requires an entire infrastructure.

First and foremost, you need the right employees.  No one can do everything.  The doctor must rely on a staff of experienced and well-trained workers.

Well, yes and no.  Additionally, everything must be done in such a way that ensures the comfort of the patient.  The office should be easily accessible, clean, and attractive in appearance, as well as have a pleasant odor, in addition to being filled with knowledgeable, competent people.  Secondly, even the most attentive and well-educated doctor needs the proper diagnostic infrastructure.  If you do not have the latest equipment, which will allow you to diagnose various medical conditions, then you simply cannot be an effective healer.  It is unrealistic otherwise.  Good intentions, attentive listening, and a pat on the back isn’t enough; you must also provide treatment.  We need to figure out what the problem is, and therefore it is necessary to constantly seek improvements in diagnostic infrastructure.

Infrastructure is often an ill-defined and overused word.  How, then, do you embody this word in your practice?

We have a large number of very complex and specific instruments.  But let’s pick up where we left off.  The next step is having a doctor that is not only well-educated, or trained under the best specialists in the field, but also one that actively teaches others.  Any physician that has ever been an instructor or teacher knows that while the students learn, the teacher learns reciprocally from their students.  They pose tricky questions, keep you on your toes, and force you to stay afloat.  Therefore, a good teacher should always be on the lookout, searching for new information in the scientific journals, attending conferences, communicating with colleagues, and participating in unions and organizations.  The process of self-improvement for a doctor is never-ending.  During the course of our education in medical school, colleges, universities, and residencies, we also learn how to continue our education as a life-long endeavor.

So is that all?  The staff and the doctor? 

The doctor must be well-educated, love to go to work, and love what he does, but this still isn’t all.  They still need experience.  Nothing in the world can replace actual experience.  Experience is the most severe of teachers, because it admonishes you before it actually teaches.  So if you don’t want to be someone’s lab rat, look for a doctor (in any medical field) who is in their prime, who is well-educated, has colleagues that they consult with…  there is the infrastructure, in the students that learn from them and the amount of experience they possess.  This combination of things allows us to offer something that few of our colleagues can afford, and that is a certain guarantee of success.

In other words, it’s a promise to put forth the best effort possible? 

That’s what a guarantee is.  However, any doctor can make a promise that they will try, as 99% of doctors are good people with good intentions.  I don’t know of any doctors that want to make their patients worse.  Everyone wants to do well, but it doesn’t always turn out like that.  Why is that?  Usually, we will evaluate their qualifications, or perhaps their lack of education, talent, experience, or equipment.  Not only do you need the latest diagnostic equipment, but also the knowledge to use it.  Take, for example, a doctor reading an x-ray.  He has the same machines, the same technology, and the same scan as other offices.  Yet some offices will read the x-ray correctly, and others will not.  Again, why is that?  That is because, at our office, we have an extremely vital, professional operator.  But you must understand that these kind of high-level, experienced operators cost money.  We cannot make a good guarantee for only a few dollars.  A lot of people are involved in a successful operation.  This is important to convey to our readers that are wondering about our difference in price.

Все водят машину и должны видеть дорогу, но у профессионального водителя будут дополнительные тонкости, а у инженера – свои конкретные жалобы. Опытный ассистент или врач должны знать, в каком направлении вести разговор. К чему я клоню? Нужно комплексное решение, и для того, чтобы получить хорошие результаты, т.е. удовлетворить жалобу пациента (потеря остроты зрения, дискомфорт, боль и т.д.), нужна целая инфраструктура.

К тому же нужно еще все  делать так, чтобы не мучить пациента.  Мало того, что офис должен быть доступным, чистым, красивым, в нем должно хорошо пахнуть, но он еще и должен быть наполнен компетентными людьми. 

, что очень важно: самому внимательному, лучшему и образованному доктору нужна дигностическая инфраструктура. Если у тебя нет новейшей аппаратуры, позволяющей диагностировать те или иные заболевания, то ты не можешь быть эффективным лекарем. Это просто нереально. Мало хороших намерений, внимательного слушанья или гладить по головке – нужно же еще и лечить. Нужно выяснить, в чем проблема. А для этого необходимо постоянно совершенствовать эту диагностическую инфраструктуру.

– Инфраструктура это такое хрустящее слово, которое много где применяют. Как именно она воплощается в вашем офисе?

– У нас имеется огромный ряд очень сложных и точных аппаратов. Но давайте по порядку. 

 – это доктор, который должен быть не только хорошо образован, не только быть обученным самыми лучшими специалистами, но это должен быть доктор, который сам обучает других. Любой врач, который когда-либо учил кого-то, знает, что студенты не только учатся, но и учат своего учителя: они задают каверзные вопросы, они заставляют тебя быть в напряжении и держаться на плаву. Поэтому хороший учитель всегда должен быть начеку, для чего необходимо читать научные журналы, ездить на конференции, общаться с коллегами, состоять в обществах. Процесс самоусовершенствования врача – безостановочный. В ходе образования, которое мы получаем в мединститутах, колледжах, университетах, резидентурах – нас учат еще и тому, как продолжать обучение на протяжении всей жизни.

– На этом все? Сотрудники и врач?

– Должен быть образованный доктор, который любит ходить на работу и любит свое дело, но и этого мало. Потому что еще нужен 

. Ничто на свете не заменит опыта. Именно опыт – самый строгий учитель, который сперва проверяет, а потом учит. И если вы не хотите, чтобы «на вас» кто-то учился, то в любой медицинской области ищите доктора, который в расцвете сил, который хорошо образован, у которого есть коллеги, с которыми он может посоветоваться, есть инфраструктура,  ученики и Опыт. Вся эта комбинация позволяет нам то, что мало кто из коллег себе может позволить, – давать пациентам определенные гарантии успеха.

– Т.е. обещание постараться?

Именно гарантии. Любой доктор вам пообещает, что будет стараться. 99% всех докторов – хорошие люди с хорошими намереньями. Я не знаю врачей, которые хотят сделать пациенту плохо. Никто намеренно плохо не делает. Все хотят сделать хорошо, но почему-то не у всех это получается. Почему? Обычно можно говорить об определенных ограничениях, нехватке образования, таланта, опыта или аппаратуры. Мало того, что нужно современное диагностическое оборудование, но нужно еще и уметь им пользоваться. Доктор, к примеру, читает рентгеновский снимок. Та же машина, та же технология, тот же скан, что и в другом офисе. Но в одном офисе прочтут правильно, а в другом нет. Почему? Потому что крайне важен профессиональный оператор, который у нас есть. А нужно же понимать, что высококлассные, опытные специалисты стоят немалых денег. Не может хорошее обеспечение стоить три рубля. Очень много людей участвует в успешной операции. Это я хотел бы донести до тех читателей, которые искренне недоумевают, почему существуют разные цены.

– А в чем конкретно состоит гарантия?

– В том, что если мы пообещали избавить вас от очков, то сделаем все, что для этого потребуется, включая бесплатный LASIK или другие опции.

⭐️ My review of LASIK after one year ⭐️I told Dr. Benjamin I would do this a while ago, but I kind of feel like it says a lot I have kept this on my checklist for a year. I feel like a year is a good amount of time to review a procedure like this and give my honest thoughts and recommendation.So, let me tell you about my journey to getting LASIK and how I ended up in the amazing care of Dr. Benjamin. After going to an optometrist for years and him advising against LASIK because of my genetically dry eyes, I finally got a second opinion from an ophthalmologist (Dr. Benjamin). I didn’t know the difference between an optometrist vs. opthalmologist, but once I learned it kind of made sense why my optometrist didn’t want me getting LASIK lol. Because he couldn’t sell me on contact lenses or glasses for the foreseeable future if I got my vision corrected :) I mean, who knows, but I am glad I got a second opinion.I figured being in Los Angeles with some of the best doctors in the world… if anyone is going to touch my big, blue eyeballs, might as well get it done while I’m here. I found this business due to the reviews and they seemed to really know what they were doing and how to talk to clients.Upon meeting Dr. Benjamin he told me that I wasn’t a candidate for LASIK with my dry eyes, but he could put me through treatments for dry eyes and reevaluate after; and that these treatments had a good success rate for most.So, I started doing two treatments every other week for about 2.5 months - IPL laser treatment and Lipiflow. Lipiflow, especially, was a weird painless experience but highly recommend anyone with dry eyes to ask about it regardless of LASIK. I could tell my eyes felt way less dry a couple days after each treatment. There are videos on YouTube that talk about this treatment… Who knew this stuff existed. I’m not gonna lie, it’s an investment to do all these treatments on top of LASIK, but my eyes rarely feel dry anymore so if you’re going to get it done - do it right.Finally, I was approved for LASIK. I ended up waiting until the end of year to make sure I saved up the funds to do it and had some time to heal. I also took advantage of an end of year deal they were offering. $1000 off was a pretty nice forcing function to get me to finally do it after prepping my eyes with the treatments. The procedure itself I couldn’t feel anything even though I was awake the entire time (medicated). Even the scary part when bubbles form on your eyes Dr. Benjamin let me know about in advance so when it happened I just went with the flow. I think the worst part was the recovery the few days after and coming home drugged up lol. I remember day 1 thinking omg what did I do to my eyes. My eyes just hurt and felt weird, but omg I could see 20/15 immediately. And then, day by day it gradually got better. It took a few weeks for my eyes to heal, but after 1 month you couldn’t even tell I had surgery. One eye was a bit more bloodshot than the other.I can see 20/15. It’s been life changing for the past year not having to worry about contacts or glasses. I just got married and was able to walk down the aisle with clear vision. I only wish I had the means to have done this earlier in life. If you’re considering it - do it! And definitely consider Dr. Benjamin to do it right.Lastly, to continue treating my dry eyes, Dr. Benjamin gave me an extra LipiFlow treatment, and also recommended I try punctal plugs (painless plugs inserted into your lids that help your tears last longer). He placed the plugs in my eyes under 5 mins. Wild.I heard from people during the LASIK process they have been nervous to get the procedure so I wrote this to help and because I think highly of this business for changing my life. And, some of the dry eye treatments I would’ve never known about had it not been for them. Again, I wish I would have had been able to do this years ago. It is so, so worth it!Thank you, Dr. Benjamin (and staff!)

Concerned about a reoccurring virus, I text Dr Benjamin’s office first thing in the morning seeking immediate advice and an appointment. In quick time I received both sage advice and a same day appointment. Dr B takes a personal interest in his patients well being and has diligent, competent and attentive staff to support the care and support needed. Can’t speak highly enough.

Been going to Dr B for nearly a decade after trying many other eye doctors in LA.  Beyond solving my chronic dry eye and being the most knowledgeable, he is an all-around great guy and spends time educating. The staff is wonderful as well.

The ophthalmologist that performed my surgery, Dr. Benjamin, was extremely skilled and knowledgeable. He took the time to thoroughly explain the procedure and answer all of my questions. I felt confident in his abilities and knew that I was in good hands.

Dr Benjamin and Alba were great. He is very professional, kind and with clear explanations. I had an eyelid quick surgery and I’m just missing the glasses prescription, I left the office and forgot to ask for it. But he and his team were amazing!
