Cataract surgery has evolved over the decades. Here's a brief history:

While ultrasound was revolutionary, it wasn't flawless. The procedure sends waves that can't be contained, potentially impacting the eye's vulnerable tissues. The cornea, particularly its inner endothelial cells, is especially sensitive. Damage to these cells can result in the cornea becoming swollen and cloudy, leading to vision resembling looking through fogged glass. Unfortunately, these endothelial cells don't regenerate once destroyed.

Recognizing the limitations of ultrasound, the medical community sought alternatives. The 

 emerged as a superior method about a decade ago. It operates with unmatched precision, targeting only the affected area, ensuring surrounding eye tissues remain unharmed.

Laser surgery's precision, combined with a surgeon's expertise and computer-assisted measurements, minimizes complications. The procedure turns cataracts into a gel-like consistency and opens the capsule with unparalleled accuracy. This laser-based method even allows for 

 correction, making minuscule incisions that self-heal, eliminating the need for stitches.

Although laser surgery has proven its efficacy, its adoption wasn't instantaneous. С ЧЕГО НАЧИНАЕТСЯ КАТАРАКТА

Как человек обычно узнает о проблеме? Он много лет ходит к оптометристу, и наступает момент, когда тот разводит руками: «Ни очками, ни контактными линзами я больше вам помочь не могу, пора к офтальмологу». Как правило, предварительный диагноз уже сделан, и в 90% случаев подтверждается: катаракта. При этом часто мы находим проблемы, не замеченные оптометристом: диабетическую ретинопатию, перерождение желтого пятна (macular degeneration), глаукому. И когда катаракта уже дошла «до ручки», пациент решает: делать операцию или нет.

Есть четыре назначения на катаракту. Самая частая причина: «севшее» зрение. Водитель плохо видит в ночное время, фотограф, художник или дантист перестают видеть то, без чего их профессия невозможна. Ну или человек просто хочет лучше видеть. Вторая причина – когда катаракта начинает опухать, отекать, что ведет к повышенному давлению и глаукоме. Это не тот случай, когда «хочешь – не хочешь», здесь нужна операция. 

Третья причина – когда доктору нужно увидеть глазное дно для лечения глаукомы или отслойки сетчатки, но из-за мутной катаракты его не видно, что угрожает здоровью глаза и может вызвать необратимую потерю зрения. 

Четвертая – катаракта уже настолько запущенная, что начинает разлагаться внутри глаза. Три последние причины – это где-то 10% случаев, а 90% – элементарное желание пациента улучшить зрение. 

Что происходит потом? У нас в Глазном Центре такой принцип: пациент должен видеть то, что видим мы. Мы вкладываем большие ресурсы в приобретение обеспечивающих визуализацию машин (medical imaging), которые позволяют пациентам увидеть катаракту в сравнении с нормальным хрусталиком, собственную сетчатку и т.д. И мы объясняем как это все работает или перестает работать. И на следующем этапе вы решаете: да, я хочу операцию, или нет, я буду жить, как раньше. Если выбрано первое, нужно решать, как именно делать.

Как выглядела операция 50 лет назад? Делался большой надрез на 180 градусов, т.е. почти на полглаза, и хрусталик выдавливался подобно косточке из вишни. Это требовало, как вы помните по своим бабушкам-дедушкам, 5-10-дневной госпитализации. Пациенты лежали без лишних движений, ожидая заживления ран от швов после надрезов. Потом произошла революция под названием «Ультразвук». Оказалось, что он может дробить хрусталик, как леденец. Процедуру назвали факоэмульсификацией, а изобрел этот метод доктор Чарльз Кельман. Как-то раз стоматолог чистил ему зубы ультразвуком, который отбивает камни, не разрушая зуб, и Кельмана озарило: а что если через надрез раздробить катаракту внутри глаза?

На протяжении сорока последующих лет ультразвуковые иглы для дробления катаракт были последним словом науки и техники. Надрезы постепенно уменьшались: 12 мм, 7 мм, и вот уже лет десять – 2-миллиметровый надрез, через который внедряется интраокулярная линза, т.е. искусственный хрусталик.

ПРОБЛЕМЫ С УЛЬТРАЗВУКОМ

И вроде все было хорошо, но не совсем. Ультразвук не останавливается на разрушенной катаракте, и продолжает движение по глазу. Подобно брошенному в пруд камню, волны от которого бегут до берега, нельзя остановить и звуковые волны.

В глазу много уязвимых тканей, таких как сетчатка, зрительный нерв, но самая чувствительная – роговица. Поэтому не такое уж редкое осложнение после ультразвука – декомпенсация роговицы. На внутренней ее стороне есть так называемые эндотелиальные клетки, которые постоянно из нее удаляют жидкость. Роговица – как корабль, в котором все время образуется течь, и есть насос, который эту воду выкачивает. Если насос сломался, судно тонет. Если не выкачивать из роговицы влагу, она отечет и помутнеет.

КЛЕТКИ НЕ ВОЗВРАЩАЮТСЯ

Клетки эндотелия, раз умерев, не возрождаются. Все мы в детстве слышали или читали в «Науке и жизнь», что нервные клетки не восстанавливаются. Теперь вы знаете, что не только нервные. Мы рождаемся с 4 тысячами клеток на квадратный миллиметр. Когда их остается менее 1800, роговица начинает отекать, и этот отек – вещь крайне опасная. Во-первых, видно, как сквозь сильно запотевшее стекло; а во-вторых, глазная боль. Вызванную ультразвуком проблему приходится решать пересадкой роговицы, а это уже инвазивная операция, донорская ткань и высокий риск. Не лучше ли без этого обойтись? 

Да, в руках хорошего хирурга риск любых осложнений – конечно, меньше. Но против физики нет приема. Если вам за 70, т.е. вы и так уже потеряли массу эндотелиальных клеток, то, идя на ультразвук, вы серьезно рискуете. Обычно у 70-75-летних уже не 4 тыс. клеток на миллиметр, а 2 тыс. Не так уж много нужно ультразвука, чтобы сбить с роговицы эти клетки и вызвать декомпенсацию. И тут даже у самого хорошего хирурга могут быть осложнения. Хотя не столько у него, сколько у вас.

РЕВОЛЮЦИЯ: ЛАЗЕРНАЯ ОПЕРАЦИЯ

Несмотря на то, что факоэмульсификация позволила делать маленький надрез и добиться хорошего зрения сразу после операции, было понятно, что рано или поздно появится лучший метод. И лет 10 назад случилась новая революция: изобрели фемтосекундный лазер, который раздробляет катаракту лучом света. Лазер, который работает тихо, элегантно, беззвучно, без вибрации и тепла. При этом зона поражения микроскопична – 5 микронов. Т.е. за пределами этих 5 микронов – нет никаких воздействий на окружающие ткани глаза.

Лазерная операция, проведенная под чутким руководством хирурга, с одной стороны, и под контролем компьютерных измерений, с другой, сводит риск осложнений почти к нулю. Катаракта под лазером превращается в желе, а капсула открывается с такой точностью, словно вы работали циркулем, хотя никакому циркулю такое не снилось. Корригируется астигматизм, мельчайшие надрезы делаются с такой точностью, что сами собой закрываются, и не нужны никакие швы.

Все это вывело операцию катаракты на совершенно другой уровень. Но не всякая технологическая революция сразу и всеми принимается. 10 лет назад было трудно поверить, что, заплатив 2 млн долларов за лазер, к тому же выплачивая лазерной компании по полторы тысячи долларов за каждый удар этим лазером, врач вообще сможет остаться на плаву. 

Какая была первая реакция? «Нас просто хотят обмануть! Вот я – такой великий хирург и такой волшебник, что мне не нужны эти ваши лазеры. Я и ультразвуком сделаю так, что любо-дорого смотреть. Ультразвук непревзойден». Я и сам примерно так вначале думал. И в то время на конференциях шли горячие дебаты, где 90% участников-консерваторов спорили с 10%-прогрессистами. Консерваторы говорили: «Вы дураки, купившие баснословно дорогую машину, чтобы делать то же самое, что мы, кудесники ультразвука, делаем и так. Нужно иметь навыки и талант, а не новые машины!» А 10% отвечали: «Ребята, да ведь 30 лет назад ваши предшественники говорили то же самое про ультразвук. Они кричали, что они сами с усами, они хирурги от Бога, которые умеют руками зашивать огромные надрезы без этого вашего ультразвука».

В нашем хирургическом центре Беверли Хиллз всё должно быть – и есть – самым передовым на западном полушарии. От врачей до оборудования. Так что у нас появился даже не один, а два лазера от двух конкурирующих компаний, каждый по 2 млн долларов. И пока мы делали лазерные процедуры на протяжении уже почти 10 лет, многие из нас начинали потихоньку собирать данные и сравнивать результаты.

Один врач делает надрез новеньким, остреньким, стерильненьким лезвием, другой – то же самое лазером. Тут ты выкатил пациента на свет – и он в восторге, и там ты выкатил пациента – и он в восторге. И что? Но давайте-ка посмотрим, что будет через неделю, месяц, полгода, 10 лет. И на итоги нельзя закрывать глаза. Уже невозможно с чистой совестью заявлять, что нет никакой разницы. Потому что результаты – налицо для хирурга, и на лице – для пациента.

При лазерной операции риск меньше В РАЗЫ. Причем «в разы» – это в десятки раз. При лазере ничтожен риск декомпенсации роговицы, гораздо лучше результаты остроты зрения. Кстати, многие не знают, что у 50-60% выходцев из СССР слабая цилиарная связка, т.е. зонулярные «подвески», на которых держится хрусталик. Поэтому после 45-50 лет нередко развивается псевдоэксфолиативный синдром: хрусталик начинает болтаться. И если при таких условиях вашу разболтанную капсулу и слабые подвески начинают молотить ультразвуком, то осложнения гарантированны. Даже если оперировал хороший хирург, в среднем через 9 лет ваш хрусталик будет «висеть на волоске». И придется делать сложную двухчасовую операцию, которая в этом городе по силам лишь нескольким хирургам, и один из них я. При этом учтем, что пациенту уже не 75, а под 90. Но если операция была сделана лазером, то даже при слабых подвесках и оскудевших запасах эндотелиальных клеток – у вас все будет нормально.

Поэтому с 1 января я сделал выбор. Я больше не желаю предлагать пациентам операцию, которая мало того, что не является лучшей, но еще и несет повышенный риск. Некоторые ездят на машине непристегнутыми, я этого не делаю никогда, и детей без крепления не вожу. И если уж вы доверяете мне как специалисту, то имейте в виду и разницу между операциями, которую нельзя игнорировать. Да, есть врачи, убедившие себя в обратном и даже утверждающие, что ультразвук лучше. Я говорю на это твердое НЕТ, поскольку принцип «Не навреди» пока никто не отменял. Глаза – не зубы, и вы не купите себе новый глаз вместо потерянного и за миллиард. Цена лазерной операции не столь высока, чтобы кто-то не мог себе ее позволить.

Если вы приходите к нам со словами: «Доктор, я хочу лучше видеть», то как я могу преднамеренно сделать вам не лучшую из существующих операций? Но если вы ее не хотите, то я даю список других врачей. Они хорошие люди и хотят как лучше, несмотря на то, что получается не всегда. При этом мы в Benjamin Eye Institute понимаем, что не для всех эти $2 тыс одинаковы, поэтому для русскоязычных пациентов есть всяческие скидки, рассрочки и пр. Стоит подумать даже не дважды, а семижды, прежде чем принять решение о ваших бесценных глазах. И наша цель тут совпадает с вашей: добиться такого зрения, которое делает человека счастливым.

⭐️ My review of LASIK after one year ⭐️I told Dr. Benjamin I would do this a while ago, but I kind of feel like it says a lot I have kept this on my checklist for a year. I feel like a year is a good amount of time to review a procedure like this and give my honest thoughts and recommendation.So, let me tell you about my journey to getting LASIK and how I ended up in the amazing care of Dr. Benjamin. After going to an optometrist for years and him advising against LASIK because of my genetically dry eyes, I finally got a second opinion from an ophthalmologist (Dr. Benjamin). I didn’t know the difference between an optometrist vs. opthalmologist, but once I learned it kind of made sense why my optometrist didn’t want me getting LASIK lol. Because he couldn’t sell me on contact lenses or glasses for the foreseeable future if I got my vision corrected :) I mean, who knows, but I am glad I got a second opinion.I figured being in Los Angeles with some of the best doctors in the world… if anyone is going to touch my big, blue eyeballs, might as well get it done while I’m here. I found this business due to the reviews and they seemed to really know what they were doing and how to talk to clients.Upon meeting Dr. Benjamin he told me that I wasn’t a candidate for LASIK with my dry eyes, but he could put me through treatments for dry eyes and reevaluate after; and that these treatments had a good success rate for most.So, I started doing two treatments every other week for about 2.5 months - IPL laser treatment and Lipiflow. Lipiflow, especially, was a weird painless experience but highly recommend anyone with dry eyes to ask about it regardless of LASIK. I could tell my eyes felt way less dry a couple days after each treatment. There are videos on YouTube that talk about this treatment… Who knew this stuff existed. I’m not gonna lie, it’s an investment to do all these treatments on top of LASIK, but my eyes rarely feel dry anymore so if you’re going to get it done - do it right.Finally, I was approved for LASIK. I ended up waiting until the end of year to make sure I saved up the funds to do it and had some time to heal. I also took advantage of an end of year deal they were offering. $1000 off was a pretty nice forcing function to get me to finally do it after prepping my eyes with the treatments. The procedure itself I couldn’t feel anything even though I was awake the entire time (medicated). Even the scary part when bubbles form on your eyes Dr. Benjamin let me know about in advance so when it happened I just went with the flow. I think the worst part was the recovery the few days after and coming home drugged up lol. I remember day 1 thinking omg what did I do to my eyes. My eyes just hurt and felt weird, but omg I could see 20/15 immediately. And then, day by day it gradually got better. It took a few weeks for my eyes to heal, but after 1 month you couldn’t even tell I had surgery. One eye was a bit more bloodshot than the other.I can see 20/15. It’s been life changing for the past year not having to worry about contacts or glasses. I just got married and was able to walk down the aisle with clear vision. I only wish I had the means to have done this earlier in life. If you’re considering it - do it! And definitely consider Dr. Benjamin to do it right.Lastly, to continue treating my dry eyes, Dr. Benjamin gave me an extra LipiFlow treatment, and also recommended I try punctal plugs (painless plugs inserted into your lids that help your tears last longer). He placed the plugs in my eyes under 5 mins. Wild.I heard from people during the LASIK process they have been nervous to get the procedure so I wrote this to help and because I think highly of this business for changing my life. And, some of the dry eye treatments I would’ve never known about had it not been for them. Again, I wish I would have had been able to do this years ago. It is so, so worth it!Thank you, Dr. Benjamin (and staff!)

Concerned about a reoccurring virus, I text Dr Benjamin’s office first thing in the morning seeking immediate advice and an appointment. In quick time I received both sage advice and a same day appointment. Dr B takes a personal interest in his patients well being and has diligent, competent and attentive staff to support the care and support needed. Can’t speak highly enough.

LASIK Eye Surgery- The short is go see Dr. Benjamin!I visited Dr. Benjamin Eye Institute after deciding I wanted to learn more about Lasik eye surgery and what the process would look like. I stare at computer screens for very long periods of time so I was nervous to make this life change but ready to ditch my glasses for good! The entire team from day 1 of my consultation made me feel very comfortable. The office is clean the staff are extremely welcoming, friendly and all took the time to answer any questions I had about the procedure - which was very quick and painless the day of. They also have awesome social content for educational purposes and just to make you laugh!I am about a month post procedure and it has been life changing and a freedom restored.
