Eyes:The Art of Choosing the Right Office

Fortunately, eye themes (as opposed to political) are not infinite, so it may appear that we are always talking about the same thing. On the other hand, though, human memory isn’t unlimited, especially when everyone is being overwhelmed with a lot of useless information. So you have to repeat it. We’re going to feature excerpts from our many conversations with Dr. Arthur Benjamin. I hope that reading these peaceful lines (excluding the fighting spirit needed to combat cataracts, glaucoma, and other enemies of the human race) will offer a respite from the recent political pandemonium.

Let’s talk about the scourge of the elderly – cataracts. I don’t know what prevented the Creator or evolution from making us so that we die in excellent physical shape, singing beautiful songs. Everything grows old, except the spirit – I found this out one gorgeous night when instead of one moon, I counted six. But I decided not to run to the ophthalmologist and instead seek alternative treatment. I read a description of these experimental drops on the internet, which had yielded good results in dogs. Mankind, as I recall, did not come from dogs, but you can’t be too sure… I ordered them and used them religiously. That was my first mistake. My second was just going to a random ophthalmologist around the corner. Why didn’t I know about you then, Dr. Benjamin?

Well, I don’t have an answer for that. Perhaps it’s because you didn’t ask for references, or wanted to save on gas. But to find your way into an office on the first attempt – that’s an art.

Even though your office was closer to my house. That’s how it happens – we do something stupid and then pay the price for it years after. And then with the other eye, I again went to a random doctor. Only after that did I come to you, I confess, to repair the damage. Otherwise, I would have needed three pairs of glasses. But anyway… Like the child prodigy I am, I got cataracts at age 46. Is this impossible to stave off? For example, they say that the antioxidants vitamin C and E can prevent the development of cataracts. What do you think?

As a journalist, you’ll appreciate the ambiguity. It may prevent them, but it may not. It may help and it may not. I’ve seen links to data that suggest the carotenoids lutein and zeaxanthin, which are also antioxidants, can defend against cataracts.

But then they may not, I get it. In other articles, they say that vitamin C reduces the risk of cortical and nuclear cataracts. Results also show that people who take vitamin C and E supplements for more than 10 years can slow the development of nuclear cataracts. 10 years???

I could refer to another. In Europe, they conducted research which showed that the two vitamins you mentioned can lead to a small reduction in the development of cataracts. But you need to remember two things: First of all, the only treatment for cataracts is a medical intervention, where the darkened lens is removed and an artificial intraocular one is installed. Secondly, cure-alls don’t exist, and the slightest defects in vision are hard to miss with cataracts. You don’t need to read popular articles, or drink 100g of carrot juice and forget about it. Just come to our office, which features unique equipment and a massive amount of experience. That’s your wisest choice.

About that equipment… What is this shiny instrument?

An aberrometer. As you can tell from the name, it measures distortion. Light passes through the cornea, iris, and lens, before falling onto the retina, where it is reflected. This device has sensors that read this reflection, known as the wavefront. When the light returns, the sensors check for distortion, but objectively rather than subjectively, thus making measurements.

With this device, is anything left to doubt? 

You don’t have to ask patients about anything?

You always should ask questions, but there aren’t answers for all of them. Exact answers depend on the success of the whole operation. It happens like this: usually, a patient comes in with horrible cataracts, through which light already cannot penetrate. That is, there’s no way to accurately measure all parameters, which we need in order to calculate for the lens.

– Есть, оказывается, еще пассионарные читатели, которые задают вопросы. Циля Альтшулер спрашивает: откуда вообще берется катаракта и все ли катаракты одинаковы? Может ли наука что-то сделать для того, чтобы она (катаракта) не возникала? Что на этот счет думает доктор Бенджамин?

– Катаракта развивается, когда протеиновые структуры в глазном хрусталике выходят из строя, что приводит к его помутнению, т.е. частичной потере прозрачности. В зависимости от локализации в хрусталике, есть три вида катаракты: нуклеарная, кортикальная и субкапсулярная. Существует несколько факторов в «рождении» катаракты, с которыми мы ничего не можем сделать. Это возраст, наследственность и этническое происхождение. Афроамериканцы, например, находятся в группе повышенного риска получить слепоту от катаракты. Также среди женщин риск несколько выше, чем среди мужчин. 

 Мужчинам-катарактникам от этого легче не становится.

– От статистики вообще никому легче не становится. Но есть общеоздоровительные факторы, которые мы отчасти можем контролировать, меняя поведение. Сюда входят: курение, точнее, некурение, уменьшение солнечного облучения (темные очки, шляпы), контроль таких заболеваний, как диабет. Но факторы это не причины, и четко доказанной связи между названными факторами и возникновением катаракты на сегодняшний день 

Читатель, подписавшийся инициалами Г.Ф., спрашивает, есть ли витамины, которые могут предотвратить катаракту? Я, кстати, где-то читал, что антиоксиданты, имеющиеся в витаминах C и E, якобы могут локомотив "Катаракта" притормозить. Неужто и вправду могут?

– Как журналист вы должны замечать осторожность формулировок. "Могут препятствовать развитию катаракты", а могут и не препятствовать. Могут помочь, а могут и не помочь. Мне приходилось видеть ссылки на данные, согласно которым каротиноиды лютеина (carotenoids lutein) и зеаксантин (zeaxanthin), также явлющиеся антиоксидантами, могут защитить от катаракты. Само слово «могут» подразумевает «или не могут». С другой стороны, проведенное в Европе исследование показало, что названные вами два витамина и бета-каротин ведут к небольшому уменьшению в развитии катаракты, но я бы не сильно на это рассчитывал, потому что никакой витамин от катаракты вас не застрахует. 

 это же морковь! Да, вряд ли кто-то рискнет сказать: пейте морковный сок, это отсрочит операцию. Если бы это было правдой, то офтальмологи бы тайно уничтожали урожаи на морковных плантациях.

– Да, полдня офтальмолог делает сложнейшие лазерные операции, а полдня истребляет морковь. Об этих исследованиях все же трудно говорить серьезно. Нужно знать, какова была выборка испытуемых, всеми ли участниками выполнялись условия эксперимента, не было ли подтасовок в результатах со стороны получателей грантов, соответствовало ли исследование целому ряду других – и строжайших – научных требований. В популярных статьях всегда можно найти «статистическое подтверждение» чуть ли не для любой выдумки. Не удивлюсь, если кто-то лечит катаракту уринотерапией, заговором или птичьим молоком. К сожалению, многие доверчивые люди запускают проблему, в итоге мы нередко имеем дело со сложнейшей катарактой. 

 С другой стороны, Национальный Институт по Исследованию Рака и Министерство сельского хозяйства США выпустили совместную рекомендацию, согласно которой потребление пяти порций фруктов и овощей каждый день обпеспечивают более, чем 100 мг витамина С и от 5 до 6 мг каротиноидов, включая упомянутые вами лютеин и зеаксантин. А две порции орехов и семян дают 8-14 мг витамина E. Вы против?

– Я обеими руками за, но с катарактой это не связано. Глаза человека – не сами по себе, это орган зрения, т.е. часть организма. Разумеется, они подвержены самым разнообразным воздействиям на организм, и определенную зависимость любого органа от качества и количества пищи, которую мы принимаем, никто не может отрицать. Просто нужно помнить, две вещи. Первая: единственное существующее лечение катаракты – это врачебное вмешательство, удаление помутневших хрусталиков и имплантация искусственных интраокулярных линз в ходе хирургической процедуры, точка. Вторая: на свете панацеи нет, и при малейших нарушениях зрения, которые при катаракте очень трудно не заметить, нужно не случайные статьи читать, не жонглировать статистикой, не ЗАЛИВАТЬ глаза морковным соком, а всего лишь прийти к офтальмологу, который реально вам поможет.

⭐️ My review of LASIK after one year ⭐️I told Dr. Benjamin I would do this a while ago, but I kind of feel like it says a lot I have kept this on my checklist for a year. I feel like a year is a good amount of time to review a procedure like this and give my honest thoughts and recommendation.So, let me tell you about my journey to getting LASIK and how I ended up in the amazing care of Dr. Benjamin. After going to an optometrist for years and him advising against LASIK because of my genetically dry eyes, I finally got a second opinion from an ophthalmologist (Dr. Benjamin). I didn’t know the difference between an optometrist vs. opthalmologist, but once I learned it kind of made sense why my optometrist didn’t want me getting LASIK lol. Because he couldn’t sell me on contact lenses or glasses for the foreseeable future if I got my vision corrected :) I mean, who knows, but I am glad I got a second opinion.I figured being in Los Angeles with some of the best doctors in the world… if anyone is going to touch my big, blue eyeballs, might as well get it done while I’m here. I found this business due to the reviews and they seemed to really know what they were doing and how to talk to clients.Upon meeting Dr. Benjamin he told me that I wasn’t a candidate for LASIK with my dry eyes, but he could put me through treatments for dry eyes and reevaluate after; and that these treatments had a good success rate for most.So, I started doing two treatments every other week for about 2.5 months - IPL laser treatment and Lipiflow. Lipiflow, especially, was a weird painless experience but highly recommend anyone with dry eyes to ask about it regardless of LASIK. I could tell my eyes felt way less dry a couple days after each treatment. There are videos on YouTube that talk about this treatment… Who knew this stuff existed. I’m not gonna lie, it’s an investment to do all these treatments on top of LASIK, but my eyes rarely feel dry anymore so if you’re going to get it done - do it right.Finally, I was approved for LASIK. I ended up waiting until the end of year to make sure I saved up the funds to do it and had some time to heal. I also took advantage of an end of year deal they were offering. $1000 off was a pretty nice forcing function to get me to finally do it after prepping my eyes with the treatments. The procedure itself I couldn’t feel anything even though I was awake the entire time (medicated). Even the scary part when bubbles form on your eyes Dr. Benjamin let me know about in advance so when it happened I just went with the flow. I think the worst part was the recovery the few days after and coming home drugged up lol. I remember day 1 thinking omg what did I do to my eyes. My eyes just hurt and felt weird, but omg I could see 20/15 immediately. And then, day by day it gradually got better. It took a few weeks for my eyes to heal, but after 1 month you couldn’t even tell I had surgery. One eye was a bit more bloodshot than the other.I can see 20/15. It’s been life changing for the past year not having to worry about contacts or glasses. I just got married and was able to walk down the aisle with clear vision. I only wish I had the means to have done this earlier in life. If you’re considering it - do it! And definitely consider Dr. Benjamin to do it right.Lastly, to continue treating my dry eyes, Dr. Benjamin gave me an extra LipiFlow treatment, and also recommended I try punctal plugs (painless plugs inserted into your lids that help your tears last longer). He placed the plugs in my eyes under 5 mins. Wild.I heard from people during the LASIK process they have been nervous to get the procedure so I wrote this to help and because I think highly of this business for changing my life. And, some of the dry eye treatments I would’ve never known about had it not been for them. Again, I wish I would have had been able to do this years ago. It is so, so worth it!Thank you, Dr. Benjamin (and staff!)

Concerned about a reoccurring virus, I text Dr Benjamin’s office first thing in the morning seeking immediate advice and an appointment. In quick time I received both sage advice and a same day appointment. Dr B takes a personal interest in his patients well being and has diligent, competent and attentive staff to support the care and support needed. Can’t speak highly enough.

Been going to Dr B for nearly a decade after trying many other eye doctors in LA.  Beyond solving my chronic dry eye and being the most knowledgeable, he is an all-around great guy and spends time educating. The staff is wonderful as well.
