So, is it a “pseudoexfoliation pseudoinfection”?

, because it’s still possible that it’s caused by a virus; we just don’t know. Take, for example, stomach cancer. It is the #1 killer in Japan, so the government requires mandatory testing. But Japanese that have come to the United States are at a much lower risk. Even Japanese that have lived here for a long time and moved back to Japan are at a lower risk. It isn’t the food, because they eat mostly the same here.

Maybe, but Okinawa on the other hand often has centenarians. There are diseases of civilization, as well. For example, Africans have no allergies and virtually no bowel cancer.

But they have tuberculosis. Returning to glaucoma, though, the upshot is that it has a single, generally-accepted reason why it occurs…

Well, it is very complex, and that leads us to the next thing. I’m a doctor with a ton of experience treating Russian-speaking patients, and I know all the different types of glaucoma including pseudoexfoliation. Our people are susceptible to glaucoma, and particularly cataracts, so this makes the operation difficult. For many physicians that encounter “our” kind of glaucoma, it’s a complete catastrophe of complications and problems. But every other patient in our office has this kind. After many years of practice, I know how to deal with it. A man may come in with normal intraocular pressure, but I can immediately notice something’s not right. Moreover, he came from Kiev…

Is some kind of prevention possible? Maybe physical activity?

There is no way to prevent it, and time determines the extent of the damage. The earlier the diagnosis, the sooner you can start treatment. In theory, no one in this day and age should go blind from glaucoma.

This happens because it’s detected too late. A patient comes in and says, “I have trouble parking the car without running into something,” and this usually means neglected glaucoma. We need to start treatment, even before visible symptoms appear. In order to diagnose glaucoma at these earlier stages, special machines are required that can detect problems 7-8 years earlier than is possible in other offices.

It’s very serious. People can take a test that says they will have Alzheimer’s. But what can you do about it? Nothing. However, if your eyes have a problem, there 

something you can do! You’re armed with knowledge and can follow the doctor’s recommendations – then, when your intraocular pressure rises, have it treated. Thus, you can alter your own destiny.

Staring at a computer screen all day – is that a factor?

Vision is a passive process, whether you’re writing a novel on the computer, knitting, or sewing. The feeling of fatigue is the drying of the eyes, and they start to tear up, redden, tingle, or burn. This is because you blink less, whether you’re looking at a monitor or a seam. Instead of 10-15 times per minute, only 5-6 times.

So in order to avoid fatigue, you need to blink more often?

Sure, but don’t neglect going to the doctor. As I’ve said many times before, a patient’s potential is limited by the doctor’s education, experience, and equipment. And there’s only one place in the world that has the newest and best… I’m kidding, but one of those places is our office, where your problems will be treated with the utmost seriousness. – Т.е. это уже как бы «псевдоэксфолиационная псевдоинфекция»?

– Может быть, и не псевдо: не исключено, что какой-то вирус это вызывает, а мы об этом пока просто не знаем. Возьмем, к примеру, рак желудка. У японцев, живущих в Японии, это киллер номер 1. Поэтому у них государство требует всеобщей проверки. Но японцы, которые перезжают в США,  

у них статистика рака желудка намного лучше. У японцев, которые жили здесь достаточно долго и потом уехали в Японию, этого тоже нет. И дело не в пище, потому что здесь они едят приблизительно то же самое.

– Влияние климата, может быть?

– Может быть, но Окинава, с другой стороны, это одно из мест, где чаще встречаются долгожители.  Есть к тому же болезни цивилизации. В Африке, например, нет аллергии и практически нет рака кишечника.

– Зато туберкулез там есть. Возвращаясь к глаукоме, получается, что единая общепринятая картина причин ее возникновения…

– … очень сложна, и это приводит нас к следующему. Я как доктор, который имеет большой опыт с русскоязычными пациентами, знаю самые разные виды глаукомы, включая псевдоэксфолиационную. У наших людей нередко характерные глаукомы, специфические катаракты,  им труднее делать операцию. Для многих врачей, когда они сталкиваются с «нашей» глаукомой 

 это целая катастрофа, масса осложнений и проблем. А у нас в офисе каждый второй пациент – с этим видом глаукомы. В ходе многолетней практики я уже наловчился справляться с ней. Приходит, скажем, человек, у него нормальное давление, но я сразу вижу, что у него что-то не так, к тому  же приехал он сюда из Киева.

– А есть хоть какая-то профилактика? Спорт помогает?

Никакой профилактики нет, степень ущерба зависит только от времени. Чем раньше делается правильный диагноз, тем быстрее ты можешь начать лечить. В принципе, никто не должен в наше время слепнуть от глаукомы.

– Не должен, но слепнет.

Это как раз и случается потому, что обнаруживается слишком поздно. Приходит пациент и говорит: я когда машину паркую, все время задеваю стену, да и без машины задеваю дверные косяки. Вот это – уже запущенный вариант. Нужно начинать лечить, когда еще ничего не заметно. А для того,  чтобы диагностировать глаукому на ранней стадии, нужно иметь специальные машины, которые могут определить проблемы на 7-8 лет раньше, чем это возможно в некоторых других врачебных офисах.

– 7-8 лет это очень серьезно, полагаю.

Это очень серьезно. Человеку сделали тесты и говорят, что у него будет Альцгеймер. Ну и что? Что ты можешь с этим поделать? А тут ты пришел, тебе сказали, что с глазами у тебя такая проблема – и у тебя есть время что-то предпринять! Теперь ты вооружен знанием и будешь следовать рекомендациям,  следить за этим, проверять, как только давление повысилось – лечить. Тем самым ты можешь изменить свою судьбу.

– А сидение за монитором сутками – это фактор?

Зрение – пассивный процесс. Неважно, пишете вы роман на комьютере, вяжете или шьете. Ощущение утомления – это сухость глаз, когда они начинают слезиться, краснеть, «колоть», «печь» и т.д. Это происходит потому, что смотрящие на монитор или на шов, реже моргают. Вместо 10-15 в минут – 5-6 раз.

– Чтобы избежать утомления, нужно чаще моргать?

Да, но стараться при этом не проморгать посещение доктора. Я это говорил уже много раз: возможности пациента в диагностике, в лечении, в результатах 

ограничены возможностями врача – какое у него образование, какой опыт и какая аппаратура. И есть только одно место на земле, где собрано все новейшее и лучшее… шучу, конечно, но одно из таких мест – наш офис, в котором к вашим проблемам отнесутся со всей серьезностью. ⭐️ My review of LASIK after one year ⭐️I told Dr. Benjamin I would do this a while ago, but I kind of feel like it says a lot I have kept this on my checklist for a year. I feel like a year is a good amount of time to review a procedure like this and give my honest thoughts and recommendation.So, let me tell you about my journey to getting LASIK and how I ended up in the amazing care of Dr. Benjamin. After going to an optometrist for years and him advising against LASIK because of my genetically dry eyes, I finally got a second opinion from an ophthalmologist (Dr. Benjamin). I didn’t know the difference between an optometrist vs. opthalmologist, but once I learned it kind of made sense why my optometrist didn’t want me getting LASIK lol. Because he couldn’t sell me on contact lenses or glasses for the foreseeable future if I got my vision corrected :) I mean, who knows, but I am glad I got a second opinion.I figured being in Los Angeles with some of the best doctors in the world… if anyone is going to touch my big, blue eyeballs, might as well get it done while I’m here. I found this business due to the reviews and they seemed to really know what they were doing and how to talk to clients.Upon meeting Dr. Benjamin he told me that I wasn’t a candidate for LASIK with my dry eyes, but he could put me through treatments for dry eyes and reevaluate after; and that these treatments had a good success rate for most.So, I started doing two treatments every other week for about 2.5 months - IPL laser treatment and Lipiflow. Lipiflow, especially, was a weird painless experience but highly recommend anyone with dry eyes to ask about it regardless of LASIK. I could tell my eyes felt way less dry a couple days after each treatment. There are videos on YouTube that talk about this treatment… Who knew this stuff existed. I’m not gonna lie, it’s an investment to do all these treatments on top of LASIK, but my eyes rarely feel dry anymore so if you’re going to get it done - do it right.Finally, I was approved for LASIK. I ended up waiting until the end of year to make sure I saved up the funds to do it and had some time to heal. I also took advantage of an end of year deal they were offering. $1000 off was a pretty nice forcing function to get me to finally do it after prepping my eyes with the treatments. The procedure itself I couldn’t feel anything even though I was awake the entire time (medicated). Even the scary part when bubbles form on your eyes Dr. Benjamin let me know about in advance so when it happened I just went with the flow. I think the worst part was the recovery the few days after and coming home drugged up lol. I remember day 1 thinking omg what did I do to my eyes. My eyes just hurt and felt weird, but omg I could see 20/15 immediately. And then, day by day it gradually got better. It took a few weeks for my eyes to heal, but after 1 month you couldn’t even tell I had surgery. One eye was a bit more bloodshot than the other.I can see 20/15. It’s been life changing for the past year not having to worry about contacts or glasses. I just got married and was able to walk down the aisle with clear vision. I only wish I had the means to have done this earlier in life. If you’re considering it - do it! And definitely consider Dr. Benjamin to do it right.Lastly, to continue treating my dry eyes, Dr. Benjamin gave me an extra LipiFlow treatment, and also recommended I try punctal plugs (painless plugs inserted into your lids that help your tears last longer). He placed the plugs in my eyes under 5 mins. Wild.I heard from people during the LASIK process they have been nervous to get the procedure so I wrote this to help and because I think highly of this business for changing my life. And, some of the dry eye treatments I would’ve never known about had it not been for them. Again, I wish I would have had been able to do this years ago. It is so, so worth it!Thank you, Dr. Benjamin (and staff!)

Concerned about a reoccurring virus, I text Dr Benjamin’s office first thing in the morning seeking immediate advice and an appointment. In quick time I received both sage advice and a same day appointment. Dr B takes a personal interest in his patients well being and has diligent, competent and attentive staff to support the care and support needed. Can’t speak highly enough.

Been going to Dr B for nearly a decade after trying many other eye doctors in LA.  Beyond solving my chronic dry eye and being the most knowledgeable, he is an all-around great guy and spends time educating. The staff is wonderful as well.

Dr Benjamin and Alba were great. He is very professional, kind and with clear explanations. I had an eyelid quick surgery and I’m just missing the glasses prescription, I left the office and forgot to ask for it. But he and his team were amazing!
