I’ve heard about the new machine – it has something to do with the seemingly mundane issue of dry eyes?

It might seem mundane, but it’s a huge problem that anyone can have, of any age. Dryness affects vision and comfort. You get the sensation there’s something in your eye, they turn red, they itch, and you start rubbing them. This causes deformity in the cornea and keratoconus may occur. In short, dry eyes are very bad. Furthermore, dryness affects the results of laser vision correction and cataract surgery. So you can understand that it worries me very much. We reach a point where the surface of the cornea starts to interfere with the achievement of more precise results.

Because when the cornea is dry, the surface is rough. It’s unpleasant for the patient and also affects the effectiveness of the procedure. Best practice suggests that you restore the surface to its normal state. How do you do that? Well, you can use drops, or we advise a warm compress. In the eyelid, there are the Meibomian glands. They secrete an oily substance that lubricates the surface of the eyeball and prevents the water in tears from evaporating. That fatty layer and water create a film. In one type of dry eyes, though, there is a lack of tears. The oily layer becomes too thick, for some reason, and the glands become clogged. As a result, they peel off, scar, and/or close.

This has already gone over the heads of most readers, so I’ll ask this – can anything be done?

It turns out, yes! Several years ago, they developed a smart machine 

with a special actuator. It warms the inside and outside of the eyelids with a stream of hot air. Only the eyelid heats up, and not the eye itself. The stream melts scars, unclogs the glands, and draws everything out of them.

The glands open! But if too much time has passed and you don’t have 5-6 functional glands, it is a useless exercise. That’s why imaging takes place beforehand to see if the patient can be helped. At the American Ophthalmology conference in Chicago, from which I recently returned, I had the procedure conducted on myself. I saw that many of my 

 are closing, which isn’t surprising at 51 years of age. I need to do something about that. If, for example, you have blepharitis, styes, or dry eyes that lead to the evaporation of tears, then you need to take action.

The takeaway is that you need to come see us. We decided that Benjamin Eye Institute cannot do without this magic machine; our reputation depends on it. And although the machine is very, very expensive ($100,000), and we have to spend $500 per patient, we already installed it and are working with it. If the patient qualifies, the procedure should be done several weeks before a cataract operation.

Once a year, or if it has been neglected, maybe twice.

I would doubt this is covered by insurance

It’s not. Nevertheless, there’s already a waiting list for these machines. I admit, I didn’t think my practice could afford such a thing, but I conducted a survey among my patients and the response was positive. In other words, they are likely to undergo this procedure since their vision depends on it. We have one goal. It was a very difficult decision to make, but we did it.

I also hope to have this procedure. Is it possible?

Даже промозглая и «ледяная» по нашим меркам погода (16 градусов тепла старыми!) не мешает пациентам собираться в похожем на зал звездолета холле.  С доктором А. Бенджамином мы, как всегда, успели затронуть несколько тем. Сегодня – первая.

– Поговорим о сказке, которая стала былью в BenjaminEyeInstitute? Она, насколько понимаю, связана с такой прозаической “болячкой”, как сухость глаз.

– Это только звучит прозаически, а на самом деле огромная проблема, от которой никто не застрахован независимо от возраста. Сухость влияет не только на зрение, но и на комфорт глаза – у вас возникает ощущение инородного тела, глаз краснеет, чешется, колется, вы его начинаете тереть, отчего деформируется роговица и может возникнуть кератоконус. Короче, сухость глаз – это очень плохо. Более того, сухость влияет на результаты лазерной коррекции зрения и результаты операции катаракты. Сами понимаете, что меня это очень сильно волнует. Мы дошли до такого рубежа, когда поверхность роговицы начинает мешать достижению более острых результатов.

– Звучит, как парадокс. Почему так происходит?

– Потому что когда роговица сухая, это означает шероховатую поверхность. Т.е. мало того, что это неприятно пациенту, но еще и влияет на эффективность процедур. Самый высокий пилотаж нашей работы предполагает, что ты  приводишь эту поверхность в нормальное состояние. Как это сделать? Да, можно капать капли, или мы советуем делать теплый компресс. Внутри века есть такие мейбомиевы железы, выделяющие сальное, маслянистое вещество, которое смазывает поверхность глазных яблок и предотвращает испарение водного компонента слез. Жировой слой и вода создают слезную пленку. Один из видов сухости глаз приводит к нехватке слезы. По разным причинам это маслянистое вещество становится слишком густым, железы забиваются и в результате одни начинают отваливаться, другие рубцуются и закрываются.

– Мы уже проходим грань, за которой вас поймут только самые сведущие читатели. Спрошу так: с этим же в любом случае ничего нельзя поделать?

– Оказалось, что можно! Несколько лет назад изобрели умную машину под названием Lipiflow со специальным «актуатором», который прогревает веки снаружи и внутри потоком горячего воздуха. При этом сам глаз не нагревается, только веко. Этот поток «расплавляет» застывшие рубцы, запекшиеся железы, и как бы сдаивает их.

– Но это, вероятно, очень долгий процесс?  

– Не очень: каких-то 12-15 минут. Плюс, конечно, подготовительный период.

– Железы открываются! Но если проблема зашла слишком далеко, и у вас не осталось хотя бы 5-6 желез, то это, увы, бесполезное занятие. Поэтому перед тем делается imaging, т.е. специальная фотография, на которой врач видит состояние желез и может решить, поможет это вам конкретно или нет. На Конференции американских офтальмологов в Чикаго, на которой я недавно был, мне самому сделали этот тест, и я увидел, что многие мои мейбомиевы железы начинают закрываться, что в общем и не удивительно в 51 год. Так что и мне самому нужно уже что-то делать. Если  вас, к примеру, мучает блефарит, если вас преследуют ячмени, если у вас сухость глаз, которая приводит к испарению слезы, то и вам нужно принимать меры, а не ждать у моря погоды.

–  Итак, сухость глаз и машина Lipiflow. Какой вывод могут сделать читатели?

– Вывод простой: нужно идти к нам. Мы в Benjamin Eye Institute решили, что без такой машины жить нельзя, поскольку наша репутация нам просто не позволяет эту волшебную машину не иметь. И несмотря на то, что она очень и очень дорогая (100 тыс долларов), к тому же на каждого пациента нужно будет тратить по 500 долларов с нашей стороны, мы все-таки ее приобрели, уже установили и с ней работаем. При наличии показаний у пациента процедуру нужно делать за несколько недель до операции катаракты. Я, признаться, даже не думал, что моя практика может потянуть такой дорогой агрегат, но, вернувшись с конференции, провел опрос среди пациентов, и очень многие дали положительный ответ – т.е. они склонны пройти через процедуру, от которой в конечном итоге будет зависеть качество их зрения. Цель-то у нас с пациентами одна. Это было трудное и очень непростое для нас решение, но мы его все-таки приняли.

⭐️ My review of LASIK after one year ⭐️I told Dr. Benjamin I would do this a while ago, but I kind of feel like it says a lot I have kept this on my checklist for a year. I feel like a year is a good amount of time to review a procedure like this and give my honest thoughts and recommendation.So, let me tell you about my journey to getting LASIK and how I ended up in the amazing care of Dr. Benjamin. After going to an optometrist for years and him advising against LASIK because of my genetically dry eyes, I finally got a second opinion from an ophthalmologist (Dr. Benjamin). I didn’t know the difference between an optometrist vs. opthalmologist, but once I learned it kind of made sense why my optometrist didn’t want me getting LASIK lol. Because he couldn’t sell me on contact lenses or glasses for the foreseeable future if I got my vision corrected :) I mean, who knows, but I am glad I got a second opinion.I figured being in Los Angeles with some of the best doctors in the world… if anyone is going to touch my big, blue eyeballs, might as well get it done while I’m here. I found this business due to the reviews and they seemed to really know what they were doing and how to talk to clients.Upon meeting Dr. Benjamin he told me that I wasn’t a candidate for LASIK with my dry eyes, but he could put me through treatments for dry eyes and reevaluate after; and that these treatments had a good success rate for most.So, I started doing two treatments every other week for about 2.5 months - IPL laser treatment and Lipiflow. Lipiflow, especially, was a weird painless experience but highly recommend anyone with dry eyes to ask about it regardless of LASIK. I could tell my eyes felt way less dry a couple days after each treatment. There are videos on YouTube that talk about this treatment… Who knew this stuff existed. I’m not gonna lie, it’s an investment to do all these treatments on top of LASIK, but my eyes rarely feel dry anymore so if you’re going to get it done - do it right.Finally, I was approved for LASIK. I ended up waiting until the end of year to make sure I saved up the funds to do it and had some time to heal. I also took advantage of an end of year deal they were offering. $1000 off was a pretty nice forcing function to get me to finally do it after prepping my eyes with the treatments. The procedure itself I couldn’t feel anything even though I was awake the entire time (medicated). Even the scary part when bubbles form on your eyes Dr. Benjamin let me know about in advance so when it happened I just went with the flow. I think the worst part was the recovery the few days after and coming home drugged up lol. I remember day 1 thinking omg what did I do to my eyes. My eyes just hurt and felt weird, but omg I could see 20/15 immediately. And then, day by day it gradually got better. It took a few weeks for my eyes to heal, but after 1 month you couldn’t even tell I had surgery. One eye was a bit more bloodshot than the other.I can see 20/15. It’s been life changing for the past year not having to worry about contacts or glasses. I just got married and was able to walk down the aisle with clear vision. I only wish I had the means to have done this earlier in life. If you’re considering it - do it! And definitely consider Dr. Benjamin to do it right.Lastly, to continue treating my dry eyes, Dr. Benjamin gave me an extra LipiFlow treatment, and also recommended I try punctal plugs (painless plugs inserted into your lids that help your tears last longer). He placed the plugs in my eyes under 5 mins. Wild.I heard from people during the LASIK process they have been nervous to get the procedure so I wrote this to help and because I think highly of this business for changing my life. And, some of the dry eye treatments I would’ve never known about had it not been for them. Again, I wish I would have had been able to do this years ago. It is so, so worth it!Thank you, Dr. Benjamin (and staff!)

Concerned about a reoccurring virus, I text Dr Benjamin’s office first thing in the morning seeking immediate advice and an appointment. In quick time I received both sage advice and a same day appointment. Dr B takes a personal interest in his patients well being and has diligent, competent and attentive staff to support the care and support needed. Can’t speak highly enough.

Been going to Dr B for nearly a decade after trying many other eye doctors in LA.  Beyond solving my chronic dry eye and being the most knowledgeable, he is an all-around great guy and spends time educating. The staff is wonderful as well.

Dr Benjamin and Alba were great. He is very professional, kind and with clear explanations. I had an eyelid quick surgery and I’m just missing the glasses prescription, I left the office and forgot to ask for it. But he and his team were amazing!
