A friend of mine recently visited Benjamin Eye Institute on account of having big floaters in his eyes, which had dramatically increased in number. As a result, he learned that he not only had problems with his retinas, as I suspected, but with diabetes as well. On this and other topics, we met with eye surgeon Dr. Arthur Benjamin. 

It seems that a visit to the ophthalmologist can offer you much more than just what’s going on with your eyes. 

I never get tired of repeating how important it is to have modern diagnostic equipment. Everyone already knows that the eyes aren’t just a window to your soul. The majority of these systemic diseases, like diabetes, hypertension, thyroidism, arthritis, and a number of others appear in the eyes, as well. Ophthalmologists are often among the first doctors to detect these diseases, and what’s more is that for people who already know that they have some kind of disorder or disease, be it diabetes or hypertension, it is still important to periodically visit the ophthalmologist.

You can check your blood sugar yourself several times a day. Why has an ophthalmologist do it?

To determine at what stage the disease is and if it is being well-controlled through treatment. An ophthalmologist can see the bigger picture because diabetes affects many organs… kidneys, lungs, and even the brain. Renal failure, heart problems and heart attacks – these are all consequences of diabetes.

In other words, a person won’t even remotely be aware that this chronic disease has reached a critical point, but you’re able to determine that for them?

Let’s say a patient goes to the doctor with diabetes. An experienced endocrinologist will remind their patients that it is necessary to go to the ophthalmologist. Why do they know to do this? The fact of the matter is that the eyes show what’s going on in the entire organism due to diabetes. Many diagnostic devices can pinpoint what’s happening in the retina. For example, our OPTOS retinal camera combs the fundus, scanning the retina, and allowing us to see the layers and levels of the retina at a high resolution. And there are other non-invasive tests that allow us to see how the blood vessels are working in the retina, the arteries, and even microscopic capillaries. But the retina, in turn, affects processes which occur in the brain, lungs, and nervous system.

So I can think that everything is ok with me, when in reality it’s not?

Fortunately, sometimes everything is just fine. Maybe not perfect, but good. But since good test results might cause you to let your guard down, the endocrinologist will sometimes advise you to see an ophthalmologist. If there are problems in the retina, then that means everything isn’t ok. That’s why you need to see the eye doctor periodically. They can not only offer the correct diagnosis, but also show you what’s really going on and possibly refer you to another doctor that isn’t an eye specialist.

Do I understand correctly that there’s a paradox, that the endocrinologist may not be able to see an endocrinological problem, but you can?

Yes, they can’t see what I see. And that doesn’t mean they’re good or bad; it’s just that they don’t have that specific equipment or the skills to use it. Just like I don’t have the skills for a clean endocrinological examination. In order for this to work like it needs to, you need not only diagnostic machines, but also what we’ve had in the office for 17 years now – an electronic filing cabinet.

We used them long before it came into vogue. There’s a file for each patient – all our tests, all our diagnostic calculations. The electronic system won’t burn up in a fire or get lost in a flood, and everything is backed up in the cloud. Even if something happens to the building, all of the data is stored. The system gives access to the patients, as well, so you can check on your test results and so on, and of course we can send these files to other doctors. This allows you to communicate effectively with your colleagues. If I see a patient who goes to the endocrinologist, it’s necessary to send the latest data on them. When you go to a new doctor, let’s say, for hypertension or high cholesterol, they’ll already have a report from the ophthalmologist, which tells them everything about your history of illness. Your new doctor promptly receives information of cardinal importance about you, so you aren’t a dark horse.

Speaking of dark horses, such was the case recently with the new technology Symfony, which you were one of the first in Los Angeles to have, like always. Remind the reader what the charm is of these new lenses, when the previous ones were already pretty good.

It’s a really revolutionary solution. The usual multifocal, artificial lenses allow great vision in the distance (mountains, driving a car, watching television) and up close (newspapers, telephones), but not so much in mid-distances – social situations, computer, speedometer, GPS. Furthermore, there are problems with haloes, glare, reflections, rays, diffusion, a loss of contrast, and a partial loss of light. People often complain, “I can see, but there isn’t enough light.” It would be nice if the transitions between distances were smooth and without loss. Until Symfony, there wasn’t a lens that could handle all these problems. There was always some kind of compromise, but now such a lens exists.

I’ve read that these new lenses have a possible defect with haloes, the same as monofocals. Are the new lenses superior in all aspects over the monofocals and old multifocals?

Yes, in this narrow sense the new lens is even better than the monofocal. Symfony is a complex diffractive lens with precise resolution. One of its main advantages, other than the mid-distance vision, is the high contrast it offers. It has proven to provide contrast in very low-light conditions. Put technically, it provides a full diffractive surface, combined with a modified, elongated shape.

Anyone can see black letters on a white board, but try looking at grey letters on a grayish-yellow page. The contrast is low and the colors may merge. The imposition of a positive aberration in the usual spherical lens on top of the positive spherical aberration of the cornea can reduce contrast, and with that the quality of vision. The new lens with its modified, elongated shape is able to compensate for the spherical aberration of the cornea.

Is the surgical procedure different than before?

We’ve already said many times that it’s not just this product or another, but also in the hands of the surgeon. Theoretically, you could buy almost any lens and have it inserted by any doctor. However, it doesn’t always turn out as you’d like. For good results, you also need first-class equipment. Even if you correctly perform the operation, but took insufficiently exact measurements, the patient will not be happy. Speaking of the new lens, I should add that it’s not just spherical but toric, which allows the ophthalmologist to rescue the patient from astigmatism without making an incision, which is fraught with problems, and promise the patient even better vision. Thanks to the elongated focus – from far away to very close – the problem of the mid-distance is brilliantly solved. Everything focuses smoothly: mountains, the sky, television, traffic signs, directions on your phone, the eyes of your beloved… a continuous range of focus!

The previous lenses have not depreciated in value?

Lenses never depreciate in value; they only become more expensive. I was the first in Los Angeles to implant a multifocal lens, and that was May 5th, 2005. Since then, of course, the lenses have continued to improve. It was $1500 per eye then, and it’s almost $4000 now. Why? That’s because the price goes up with each new edition. And if you say, “Then give me the old version!”, they’ll respond, “We don’t make them anymore. We will not make a poor, obsolete product. Only the best.”

I get it… just like Apple. My iPad, for example, is now considered an uninteresting, prehistoric monster, and it’s only a few years old. And you don’t see a drop in price because this version simply isn’t available anymore. 

Again, this new lens, despite its revolutionary nature, isn’t dramatically more expensive.

It’s a pity that I already have the older model. I wish I had this rich-in-contrast supervision!

Yes, you have the good, old monofocal. But the Symfony is newer and better. As a consolation, I can tell you that these new lenses aren’t for everyone, just like any other. And in order to best select candidates for this phenomenal new lens, you need intelligent diagnostic equipment, experience, and a well-coordinated team of professionals.

Похоже, посещение офтальмолога может дать намного больше, чем “что там с глазами”?  

– Я не устаю повторять, как важно иметь совершенную диагностическую  аппаратуру в медицинском офисе. Все уже знают, что глаза – зеркало не только души. Большинство таких системных заболеваний, как диабет, гипертония, проблемы с щитовидкой, артрит и ряд других – проявляется в глазах. Часто одним из первых врачей, которые обнаруживают заболевание, оказывается именно офтальмолог. Скажу больше. Людям, которым уже известно о наличии у них того и ли иного заболевания, будь то диабет или гипертония, все равно важно периодически показываться офтальмологу.

Сахар можно и так проверять у себя несколько раз в день, зачем для этого офтальмолог?

– Чтобы определить, на какой стадии находится болезнь, достаточно ли хорошо она контролируется лечением. Офтальмолог видит более широкую картину, ведь диабет влияет на многие органы: почки, легкие и даже мозг. Потеря почечной функции, проблемы с сердцем, инфаркты – такими могут быть последствия диабета.

Т.е. человек может даже и близко не думать о том, что привычная болезнь достигла критической черты, а вы это можете определить?

– Скажем, приходит к врачу пациент с диабетом. Опытный эндокринолог напоминает своим пациентам: обязательно навестите офтальмолога. Ну зачем, казалось бы, эндокринологу знать, что видит офтальмолог? Но дело в том, что глаза показывают, что происходит со всем организмом при диабете. Разными диагностическими приборами можно «засечь», что происходит в сетчатке. Например, наша цифровая камера OPTOS «прочесывает» глазное дно, сканирует сетчатку и позволяет нам в высоком разрешении увидеть несколько слоев и уровней сетчатки. Есть и другие неинвазивные тесты, которые позволяют видеть, как работают сосуды в сетчатке, артерии и даже микроскопические капилляры. А в сетчатке, в свою очередь, отражается процесс, который происходит в мозге, в легких или в нервной системе.

Т.е. я могу думать, что у меня все прекрасно, а оно не совсем?

– К счастью, бывает, что у вас все и вправду если не прекрасно, то хорошо. Но для того, чтобы не быть убаюканным анализами, которые показывают, что «все хорошо», эндокринолог и просит показаться офтальмологу. Если в сетчатке есть проблемы, значит не «все хорошо». Вот почему глазному врачу нужно периодически показываться на глаза: он может не только навести на правильный диагноз, но и показать, что реально происходит прямо сейчас и, возможно, направит вас к другому, уже не глазному специалисту.

Я правильно понимаю, что есть тут некий парадокс, и эндокринолог может не увидеть эндокринологическую проблему, которую увидите вы?

– Да, он не увидит то, что увижу я – и совсем не потому, что он плохой или хороший, просто у него нет для этого специфических аппаратов и навыков пользования ими. Равно, как и у меня нет навыков и оборудования для чисто эндокринологического обследования. Для того, чтобы все это работало, как надо, мало иметь диагностические машины, а нужно еще иметь и то, что у нас в офисе уже 17 лет  – электронную картотеку.

– Нынче она входит в моду.

– У нас она возникла задолго до того, как вошла в моду. Там в карточке на каждого пациента – все наши анализы, все диагностические выкладки. Электронная система не горит в огне, не тонет в воде, она локализуется в «облаке». Даже если что-то случится со зданием, все ваши данные сохранятся. Система дает допуск и самим пациентам, так что вы можете проверять результаты своих тестов и прочее, ну и, конечно, мы можем давать эту карточку другим врачам. Это позволяет эффективно коммуницировать с нашими коллегами. Если я смотрю пациента, который ходит к эндокринологу, то обязательно посылаю последнему все данные. Когда вы приходите к новому врачу, скажем, с гипертонией или высоким холестеролом, в его картотеке уже есть отчет от офтальмолога, который ему рассказывает все, что известно из истории болезни. Ваш новый доктор вовремя получает кардинально важную информацию о вас, и вы для него не темная лошадка.

– К слову о темной лошадке. Еще недавно таковой была новейшая технология Symfony, которой вы, как всегда, овладели одним из первых в Лос-Анджелесе. Напомните читателю, в чем прелесть этих новых линз, если и предыдущие были довольно хороши?

– Это действительно революционное решение. Обычный мультифокальный искусственный хрусталик позволяет хорошо видеть вдали (горы, вождение машины, телевизор), и вблизи (газета, телефон), но не так хорош на средней дистанции – общение, компьютер, спидометр, GPS… К тому же есть проблемы с «нимбом», ореолом, отсветами, лучами, «распылением», потерей контрастного восприятия и частичной потерей света. Люди порой жаловались: все вижу, но как бы света не хватает. Хотелось бы, чтобы переходы между дистанциями были плавные и без таких потерь. Так вот до 

 не было хрусталика, который бы решал все эти проблемы. Всегда приходилось идти на какой-то компромисс. А теперь такой хрусталик – существует.

Я читал, что у новых линз возможная погрешность в смысле ореола 

 такая же малая, как у монофокальных. При этом новые превосходят по всем параметрам и монофокальные и старые мультифокальные?

– Да, в этом узком смысле новая линза – даже лучше, чем монофокальная. 

 –  сложнейшие дифракционные линзы с точным разрешением. Одно из самых главных достоинств, помимо средней дистанции, – это высокая контрастность, которую она дает. Линза имеет проверенную функцию контрастности и в условиях весьма низкой освещенности. Выражаясь технически, она обеспечивает полную дифракционную поверхность, сочетающуюся с модифицированной вытянутой формой.

– Не поясните про контраст?

– Любой увидит черные буквы на белом щите, но попытайтесь разглядеть серые буквы на серо-желтом листе. Контрастность мала и потому они могут сливаться. Наложение положительной аберрации в случае обычных сферических линз на положительную сферическую аберрацию роговицы может снизить контрастность, а с ним и качество зрения. Новые же линзы с их модифицированной вытянутой формой способны компенсировать сферические аберрации роговицы.

А сама процедура в хирургическом плане отличается от предыдущих?

– Мы уж не раз говорили, что дело не только в той или иной новинке, но и в руках хирурга. Теоретически любую или почти любую линзу может купить и кому-то вставить любой врач. Однако не у всех это получается так, как хотелось бы и им самим, и пациенту. Помимо рук, для хорошего результата нужно еще и первоклассное оборудование. Даже если ты правильно делаешь операцию, но у тебя недостаточно точно сделаны измерения, пациент не будет счастлив. Говоря о новом хрусталике, нужно добавить, что он бывает не только сферический, но и торический, что позволяет офтальмологу избавлять пациентов от астигматизма без всяких насечек, чреватых проблемами, и обещать им еще лучшее зрение. Благодаря удлиненному фокусу – от дали до самой «близи» –  проблема «средней дистанции» блестяще решена. Все в этом хрусталике находится в плавном фокусе: горы, небо, телевизор, дорожные знаки, адрес в телефоне, глаза любимой. Беспрерывная шкала фокуса!

Предыдущие линзы не подешевели с годами?

– Линзы никогда не дешевеют, скорее наоборот. Я первым в Лос-Анджелесе поставил мультифокальную линзу 5 мая 2005 года. С тех пор хрусталики продолжают улучшаться. Тогда это стоило 1500 долларов за один глаз, а сейчас почти 4 тыс. Почему? Потому что с каждым новым выпуском линз они начинают дорожать. И если вы скажете: «А дайте мне тогда самую старую версию!», то вам ответят: «Мы ее больше не изготовляем. Мы не можем делать устаревшее и плохое, мы делаем только самое лучшее».

Ну это понятно. Мой iPad, к примеру, нынче считается доисторическим монстром, а прошло-то с покупки лишь несколько лет. И падения цен не замечено, поскольку этой модели и нет уж нигде.

– Повторюсь, что новая линза, несмотря на революционные отличия, дороже совсем немного, не драматически.

Какая жалость, что у меня уже стоят линзы старого образца, мечтаю о контрастном суперзрении, которым хвастают ваши счастливые пациенты!

– Скажу в утешение, что новые линзы, как и любые другие, подходят не всем. И чтобы определить кандидатуру на этот новый феноменальный хрусталик, нужна умная диагностическая аппаратура, опыт и сыгранная команда профессионалов.

⭐️ My review of LASIK after one year ⭐️I told Dr. Benjamin I would do this a while ago, but I kind of feel like it says a lot I have kept this on my checklist for a year. I feel like a year is a good amount of time to review a procedure like this and give my honest thoughts and recommendation.So, let me tell you about my journey to getting LASIK and how I ended up in the amazing care of Dr. Benjamin. After going to an optometrist for years and him advising against LASIK because of my genetically dry eyes, I finally got a second opinion from an ophthalmologist (Dr. Benjamin). I didn’t know the difference between an optometrist vs. opthalmologist, but once I learned it kind of made sense why my optometrist didn’t want me getting LASIK lol. Because he couldn’t sell me on contact lenses or glasses for the foreseeable future if I got my vision corrected :) I mean, who knows, but I am glad I got a second opinion.I figured being in Los Angeles with some of the best doctors in the world… if anyone is going to touch my big, blue eyeballs, might as well get it done while I’m here. I found this business due to the reviews and they seemed to really know what they were doing and how to talk to clients.Upon meeting Dr. Benjamin he told me that I wasn’t a candidate for LASIK with my dry eyes, but he could put me through treatments for dry eyes and reevaluate after; and that these treatments had a good success rate for most.So, I started doing two treatments every other week for about 2.5 months - IPL laser treatment and Lipiflow. Lipiflow, especially, was a weird painless experience but highly recommend anyone with dry eyes to ask about it regardless of LASIK. I could tell my eyes felt way less dry a couple days after each treatment. There are videos on YouTube that talk about this treatment… Who knew this stuff existed. I’m not gonna lie, it’s an investment to do all these treatments on top of LASIK, but my eyes rarely feel dry anymore so if you’re going to get it done - do it right.Finally, I was approved for LASIK. I ended up waiting until the end of year to make sure I saved up the funds to do it and had some time to heal. I also took advantage of an end of year deal they were offering. $1000 off was a pretty nice forcing function to get me to finally do it after prepping my eyes with the treatments. The procedure itself I couldn’t feel anything even though I was awake the entire time (medicated). Even the scary part when bubbles form on your eyes Dr. Benjamin let me know about in advance so when it happened I just went with the flow. I think the worst part was the recovery the few days after and coming home drugged up lol. I remember day 1 thinking omg what did I do to my eyes. My eyes just hurt and felt weird, but omg I could see 20/15 immediately. And then, day by day it gradually got better. It took a few weeks for my eyes to heal, but after 1 month you couldn’t even tell I had surgery. One eye was a bit more bloodshot than the other.I can see 20/15. It’s been life changing for the past year not having to worry about contacts or glasses. I just got married and was able to walk down the aisle with clear vision. I only wish I had the means to have done this earlier in life. If you’re considering it - do it! And definitely consider Dr. Benjamin to do it right.Lastly, to continue treating my dry eyes, Dr. Benjamin gave me an extra LipiFlow treatment, and also recommended I try punctal plugs (painless plugs inserted into your lids that help your tears last longer). He placed the plugs in my eyes under 5 mins. Wild.I heard from people during the LASIK process they have been nervous to get the procedure so I wrote this to help and because I think highly of this business for changing my life. And, some of the dry eye treatments I would’ve never known about had it not been for them. Again, I wish I would have had been able to do this years ago. It is so, so worth it!Thank you, Dr. Benjamin (and staff!)

Concerned about a reoccurring virus, I text Dr Benjamin’s office first thing in the morning seeking immediate advice and an appointment. In quick time I received both sage advice and a same day appointment. Dr B takes a personal interest in his patients well being and has diligent, competent and attentive staff to support the care and support needed. Can’t speak highly enough.

Been going to Dr B for nearly a decade after trying many other eye doctors in LA.  Beyond solving my chronic dry eye and being the most knowledgeable, he is an all-around great guy and spends time educating. The staff is wonderful as well.

The ophthalmologist that performed my surgery, Dr. Benjamin, was extremely skilled and knowledgeable. He took the time to thoroughly explain the procedure and answer all of my questions. I felt confident in his abilities and knew that I was in good hands.

The surgeons at Benjamin Eye Institute are true experts in their field - they were able to explain the procedure in detail and answer all of my questions and concerns. The surgery itself was quick and painless, and the recovery was much faster than I had anticipated.

I just completed my first visit and can say that this office deserves every high ranking it gets. The staff are efficient, friendly but highly professional.  The office is modern and sparkling clean. Dr Benjamin and Alba were great. He is very professional, kind and with clear explanations. I had an eyelid quick surgery and I’m just missing the glasses prescription, I left the office and forgot to ask for it. But he and his team were amazing!

I was referred to Dr. Benjamin from both my eye doctor and a very close friend of mine who also had her LASIK performed there. My experience was exceptional. Every single person that you meet in the office is a joy to work with. Whether it was the constant and clear communication, or the fact that they were always listening to feedback and answering my questions and concerns, they went out of their way to make me feel comfortable trusting my vision to them. They were very kind and never once pressured me into anything (including when I had to push my surgery back after I broke my leg).I would recommend this place to anyone who has eye problems because they have the most advanced equipment and Dr. Benjamin is caring, thorough and knowledgeable.
