We spoke with Dr. Benjamin today about a new method for treating dry eyes. However, as usual, we touch on other topics, as well. Even Avicenna.

I have a bit of dust in my eye, but before you take a look let’s talk. I heard a rumor that you’ve got some new stuff?

 Yes, there’s new stuff. I attended the 5-day ASCRS convention (American Society of Cataract and Refractive Surgery). There were 20,000 ophthalmologists from all over the world, their employees, and their managers. They shared their experiences, including new approaches to old problems. Hundreds of pharmaceutical and technological companies showed their achievements, as well. One of the topics covered was the cornea. The surface of the eye is not only responsible for comfort and aesthetic, but also vision. Might I remind you that 90% of the eye’s focusing ability depends on the cornea, and the amount of nerve endings there exceeds even the most sensitive of other organs. Therefore, even the smallest speck in your eye, as in your case, can feel like an entire tree branch. Patients who complain about redness, fatigue, or pain aren’t exaggerating, and this can lead to a loss of vision.

I have a buddy, by the way, whose eyes are always watering. Should he see a doctor?

Yes. The eye is continuously irrigated with tears, which have a complex structure. They consist of three layers – oil, water, and mucous. Ideally, the tear film is distributed smoothly and evenly across the surface of the eye. It is in a similar manner that water beads on the surface of a freshly-waxed car. We don’t want that on the surface of the eye – we want uniform moisture, and the oily layer is responsible for that. It also prevents evaporation. When this layer is broken, the tears not only clump together but also quickly evaporate. This leads to dryness, the eye is irritated, and there’s the sensation of having something stuck in it. If you notice, for example, when you’re looking at the television or computer for too long, your vision starts to blur. You rub your eyes, though, and it fixes itself. This is the first sign of dry eyes.

There are several ways. The first is to replenish whatever component of the tear that’s missing. Oil and water, everyone knows, do not mix. Therefore, if you want to use eye drops, you need two different kinds – one oily and one watery. We have both at the office, and I advise patients to alternate throughout the day. The mucous layer, though, is hard to replace.

So artificial tears don’t cover everything?

The product sold in stores is usually watery. In fact, they deplete more than they replenish. Imagine your hands are dry and you splash water on them. This eventually washes away the oils that moisturize the surface. Most people who complain of watery eyes don’t know that this is usually due to dryness. “How can they be dry,” they ask, “when I’m tearing up all the time?” This is because the eye overreacts in response to dryness as the result of million of years of evolution. But it’s a vicious cycle.

Kinda like global warming, which starts to manifest in cold spells.

The more the oily layer washes away, the drier the eye becomes, and this leads to a greater discharge of tears. How do you break the paradox? Use artificial oily tears. Another method is to increase the amount of tears produced by the eye itself. Why, when we cry, does our nose run? It’s from an excess of tears. They are needed in the eyes, though, and not the nose. Thus, we can put collagen inserts into the opening of the tear ducts which retain those tears and dissolve over time.

They don’t feel like a foreign object? Does it hurt?

It’s a bioinert material that takes 30 seconds to put in. It doesn’t hurt at all. The bottom line is that it saves more of the eye’s own tears. You don’t need to constantly buy drops and remember to apply them. Women tend to like this method because it doesn’t interfere with makeup. Men appreciate it, as well, because they don’t like concerning themselves with health in the first place. The inserts are usually paid for by insurance and are quite popular.

This increases the amount of tears, but if the composition is off how do we improve its quality?

There’s a really expensive medication (around $600/mo) that’s not covered by insurance. It reduces the inflammation of the lacrimal glands, and that slightly improves the quality of tears. The process takes about 3 months to see improvement, though, and the drops are used for life. There are situations (arthritis, for example) where you can’t do without them, but this doesn’t apply to the majority of people with dry eyes, which is 30 to 70 million in this country alone. Now we’re finally getting to the point! Everything I’ve said until now is something we’ve discussed before. The oily layer is produced in the meibomian glands. They’re similar to the sebaceous glands of the skin. Over time, they regress and have blockages, inflammation, and collect bacteria, which appears on the eyelids as a slimy, eyelash-enveloping biofilm. Toxins get into the eye, and as a result you have redness, itching, and other symptoms. Neither hot compresses nor shampoo can wash it away, either.

Our readers are getting impatient. They want to hear about this new thing.

 appeared. It removes this biofilm and blockage. Let’s use a teeth analogy. We brush our teeth, use Listerine, and floss, but we still need to see a hygienist to remove the plaque that a toothbrush can’t reach. This same kind of treatment is done on the eyelashes and eyelids, but on the surface. So what do we do about the internal processes, like the shortening and atrophy of the glands? From ages 50-60, folks may not notice it, but a scanner can show their true, deplorable state.

The LipiFlow machine heats the eyelid inside and out, and extracts the blockage with a gentle massage. If you care about the health of your eyes, this should be done 4 times in the first year of treatment, and if everything goes as it should then less often in the future. Also, if you start at a younger age, it requires less treatment from the beginning – around 2 times a year.

The glands start to age between 25 and 30 years old. Warming the glands allows them to return to a clear, more youthful state, which also affects facial rejuvenation. We have been conducting this process in the office for about 7 months now, and the results are simply amazing. We recommend it before cataract surgery. Almost all infections and complications occur because of blockages in the eyelid, and antibiotics are not the best option.

So you use BlephEx and LipiFlow together, right? How do I pay for all this?

Neither Medicare nor Medicaid cover this procedure, similar to dental issues. At the conference, I listened to lectures by scientists who measured the osmolarity of tears (ie, the salinity), and the concentration of substances. Even using BlephEx without LipiFlow improves the osmolarity. Using both is even better. BelphEx clears away what LipiFlow doesn’t get to. This dramatically improves quality of vision.

But this doesn’t entirely depend on the patient, I take it?

No. Avicenna (first century Persian father of medicine) said, “There’s three of us here – you, me, and the disease. And if you are with me, together we can defeat it”. In this case, it’s more like, “There’s you, me, the disease, and insurance, and the four of us must contribute to fight the disease”. As a doctor, I do my bit – I go to conferences, share the latest knowledge, and invest resources, time, and money for the newest technology. It’s hundreds of thousands of dollars. Insurance pays for what it can, but it won’t pay for everything. You too, as a patient, are financially responsible. The idea that “I have insurance and that’s all I need” does not reflect reality. Good results are very important to us, as is our reputation, so we always try to help – but not to the detriment of our practice. We have to at least cover our expenses, even where we meet maximum requirements.

I would hope that not everyone in this country is living paycheck-to-paycheck, although many have difficulty. 

There are some patients that have no problems paying full price, but there are others without those opportunities. They need their vision, as well. When people come in, we discuss all options. We want our patients, after the procedures and operations, to have better results than they would with other doctors. We wish everyone else well, but our patients come first. That’s what BlephEx and LipiFLow are for, as well as the other smart machines and latest technology. This isn’t only a financial investment for us; it’s also in physical and mental fortitude. Both parties are satisfied with the results. Life is the same and still beautiful, despite its difficulties.

– Вначале посмотрим, потом поговорим. (после осмотра и помощи).  Да, новинки есть. Я посетил 5-дневную конференцию ASCRS (Ассоциацию хирургов  по катаракте и рефрактивной хирургии).  Там 20 тыс офтальмологов со всего мира, их сотрудники, менеджеры, делились опытом, включая новые подходы к старым проблемам. Сотни фармацевтических и технологических компаний показывали свои достижения. Одна из тем была посвящена роговице. Поверхность глаза отвечает не только за комфорт и за то, как глаз выглядит, но и за зрение. Напомню, что от роговицы зависят 90% фокусирования глаза, она богато иннервирована: плотность нервных окончаний в ней больше, чем в любых других, даже самых чувствительных органах.  Поэтому инородное тело в виде мельчайшей соринки, как в вашем случае, иногда ощущается, как бревно в глазу. Пациенты, жалующиеся на красноту глаз, усталость, на колющие ощущения, понимают, что все это не ерунда, и приводит к ухудшению зрения.

– У меня, кстати, приятель вечно ходит «в слезах», нужно ли ему к врачу?

– Нужно. Глаз постоянно орошается слезой, у которой непростая структура. Состоит она из трех слоев: маслянистого, водянистого и слизистого. В идеале слеза гладко и ровно ложится на поверхность глаза.  Но то в идеале. Если, скажем, ваша машина покрыта воском, то дождь собирается на ней в капельки – и это как раз то, чего мы не хотим на поверхности глаза.  Мы хотим равномерности, за что и отвечает маслянистый слой. И он же предотвращает испарение слезы. Когда этот слой нарушен, слеза не только собирается в сгустки, но и быстро испаряется. Это и приводит к сухости, глаз раздражен и возникает ощущение инородного тела. Если вы, к примеру, замечаете, что при смотрении на киноэкран или монитор компьютера ваше зрение начинает мутнеть, но вы потерли глаз, и вроде оно восстановилось, – это первый признак сухости глаза.

– Есть несколько методов. Первый из них – это восполнение «дефицитных» компонентов слезы.  Масло и вода, как известно, плохо сочетаются.  Поэтому если вы хотите восполнить слой наружным капаньем, то нужно иметь по меньшей мере две искусственных слезы – маслянистую и водянистую. У нас в офисе есть и те и другие, обычно я пациентам советую их чередовать их в течение дня. А вот слизистый слой восполнять сложно.

– А artificial tears – не на все случаи жизни?

– Те, которые продаются в магазинах, они обычно водянистые.  На самом деле они скорее вымывают тот состав, который должны восполнить.  Представьте, что у вас сухие руки и вы брызгаете на них водой. Это в итоге смывает все масла, которые увлажняют поверхность. Большинство людей, которые жалуются на слезоточивость, не знают, что в 90% случаев это происходит из-за сухости глаз. Как же это может быть сухой глаз, спрашивают они, если он все время мокрый? Это потому, что глаз неправильно реагирует на «дефицит». Он реагирует так, как его учили миллионы лет эволюции, но это по сути замкнутый круг.

– Чем-то похоже на глобальное потепление, которое проявляется в морозах.

– Чем больше вымывается маслянистой слезы, тем суше становится глаз, что приводит к еще большему выделению слезы. Как прервать этот парадокс? Использовать искусственную маслянистую слезу. Другой метод – увеличить количество слезы, вырабатываемой самим глазом. Почему, когда мы плачем, у нас начинает течь из носа? Потому что туда поступают излишки слезы. Но носоглотке слезы не нужны, они нужны глазу. Поэтому мы ставим коллагеновые вкладыши, своего рода «пробки» в проточные каналы.  Они со временем растворяются.

– А они не воспринимаются как инородное тело? Наверное, больно?

– Это биоинертный материал, который ставится за 30 секунд и это совершенно не больно.  Суть в том, что в результате ты помогаешь глазу сохранить больше собственной слезы.  Не нужно постоянно покупать капли и не нужно помнить о регулярном их закапывании. Девушкам этот метод нравится, потому что не мешает макияжу, а мужчины, как известно, вообще не любят лишний раз заботится о здоровье, так что ценят и они. «Пробки» эти оплачиваются страховкой и весьма популярны.

– Этот метод повышает количество слезы. Но если структура нарушена, как улучшить ее качество?

– Есть очень дорогие лекарства (примерно $600 в месяц), не покрываемые страховкой.  Они снижают воспалительные процессы слезных желез, что немного улучшает качество слезы. Но процесс  относительного улучшения занимает три месяца, и капать их нужно всю жизнь. Бывают ситуации, артриты, например, когда без них не обойтись. Но это не распространяется на большинство людей с сухостью глаза, число которых в этой стране – от 30 до 70 млн. И тут мы наконец подходим к главному! Все что я сказал до этого, мы обсуждали раньше. Итак, маслянистый слов вырабатывается в так называемых мейбомиевых железах. Они аналогичны сальным железам на коже.  Со временем они регрессируют и образуются закупорки, воспаления, на веках появляются бактерии, которые (как и на зубах) образуют склизкую, обволакивающую ресницы биопленку – налет своего рода. Токсины попадают в глаз, и в результате вы имеете красноту, зуд и прочие симптомы. Ни горячие компрессы, ни детский шампунь не могут этот налет смыть.

– Читатель нетерпеливо ждет, когда вы опишете новинку.

– Недавно появился еще один инструмент – BlephEx, с помощью которого, представьте, снимается эта  биопленка и закупорки. Если продолжить аналогию с зубами: мы чистим зубы, полоскаем рот Листерином, используем нитку, но все равно нужно идти к гигиенисту и снимать налет, который не поддается зубной щетке.  Такое же лечение проводится на ресницах и ребре века. Но это на поверхности. А что делать с внутренними процессами  – укорочением и атрофированием желез, которые из-за закупорок выглядят, как песочные часы?  В возрасте 50-60 лет люди еще ничего не замечают, но сканер показывает истинное, порой плачевное состояние их желез.

– Машина LipiFlow прогревает веко внутри и снаружи, и мягким массажем сцеживает растопленные выделения. Поэтому если вам дороги ваши глаза, то эту процедуру нужно делать 4 раза в первый год, и дальше, если все пойдет как нужно, – реже. А если начать в молодом возрасте, то можно реже с самого начала – с двух раз в год.

– А что мы понимаем под молодым возрастом?

– Процесс старения желез начинается у 25-30-летних. Прогревание позволяет железам вернуться в свое прочищенное, более юное состояние, что сказывается и на омоложении лица. Мы делаем эту процедуру у нас в офисе где-то 7 месяцев и результаты просто потрясающие.  Теперь мы рекомендуем обязательно ее делать перед операцией катаракты.  Чуть ли не все инфекции и осложнения происходят лишь потому, что у пациента плохие веки.  Заливание антибиотиками – не лучший вариант.

– Итак, BlefEx  и Lipiflow. Они, видимо, сочетаются? А как за все это платить?

– Ни Медикэр, ни Медикал (опять же, как и с зубами) не покрывают этих процедур. На упомянутой конференции я слушал лекции ученых, которые измеряли осмолярность слезы, т.е. ее «соленость», концентрацию веществ.  Даже сделав чистку на BlefEx без Lipiflow, мы улучшаем осмолярность. А сделать и то, и другое – еще лучше. BlefEx счищает то, до чего не добирается Lipiflow. Это сказочно отражается на качестве зрения.

– Но не все зависит от вас?

– Не все. Авиценна говорил: 

«Здесь нас трое: ты, я и болезнь. И если ты за меня, то вместе мы ее победим»

. В нашем случае получается так: 

«Нас четверо: вы, я, болезнь и ваша страховка”.

 И мы трое должны делать взнос в ваше здоровье против болезни. Я как врач вношу свою лепту: езжу на конференции, плачу за получение новых знаний,  вкладываю ресурсы, время и деньги в покупку новейших машин и технологий за сотни тысяч долларов. Ваша страховка платит за что может, но не может платить за все. Но и вы тоже, как пациент, несете финансовую ответственность. Все трое должны объединить свои вклады.  Идеологема «У меня есть страховка, мне больше думать не о чем» – не отражает реальность. Да, люди должны делать выбор. И мы, понимая, что многие русские пациенты ограничены в ресурсах,  стараемся им помочь, и даем им серьезные (40-50%) скидки. Для нас крайне важны хорошие результаты, очень важна репутация, мы всегда стараемся помочь людям, но не можем это делать в ущерб нашей практике. Мы должны хотя бы минимально покрывать наши расходы даже там, где максимально идем навстречу.

– Я надеюсь, не все люди в этой стране едва сводят концы с концами. Хотя бывают сложности.

– Есть пациенты, которые могут себе без проблем позволить полную цену, и есть другие с меньшими возможностями, но с такой же потребностью хорошо видеть. Люди приходят, и мы обсуждаем все возможные варианты. Мы хотим, чтобы наши пациенты после процедур и операций видели лучше, чем у других врачей.  Другим  мы тоже желаем только хорошего, но в первую очередь нас заботят наши пациенты.  Вот для чего и BlefEx,  и Lipiflow, десятки других умных машин и новейших технологий. 

Хотелось, чтобы все понимали, что мы тратим на это не только финансовые, но и физические, и душевные силы. И результатами, нужно признать, довольны обе стороны.  Жизнь по-прежнему одна и по-прежнему прекрасна, несмотря на сложности.

⭐️ My review of LASIK after one year ⭐️I told Dr. Benjamin I would do this a while ago, but I kind of feel like it says a lot I have kept this on my checklist for a year. I feel like a year is a good amount of time to review a procedure like this and give my honest thoughts and recommendation.So, let me tell you about my journey to getting LASIK and how I ended up in the amazing care of Dr. Benjamin. After going to an optometrist for years and him advising against LASIK because of my genetically dry eyes, I finally got a second opinion from an ophthalmologist (Dr. Benjamin). I didn’t know the difference between an optometrist vs. opthalmologist, but once I learned it kind of made sense why my optometrist didn’t want me getting LASIK lol. Because he couldn’t sell me on contact lenses or glasses for the foreseeable future if I got my vision corrected :) I mean, who knows, but I am glad I got a second opinion.I figured being in Los Angeles with some of the best doctors in the world… if anyone is going to touch my big, blue eyeballs, might as well get it done while I’m here. I found this business due to the reviews and they seemed to really know what they were doing and how to talk to clients.Upon meeting Dr. Benjamin he told me that I wasn’t a candidate for LASIK with my dry eyes, but he could put me through treatments for dry eyes and reevaluate after; and that these treatments had a good success rate for most.So, I started doing two treatments every other week for about 2.5 months - IPL laser treatment and Lipiflow. Lipiflow, especially, was a weird painless experience but highly recommend anyone with dry eyes to ask about it regardless of LASIK. I could tell my eyes felt way less dry a couple days after each treatment. There are videos on YouTube that talk about this treatment… Who knew this stuff existed. I’m not gonna lie, it’s an investment to do all these treatments on top of LASIK, but my eyes rarely feel dry anymore so if you’re going to get it done - do it right.Finally, I was approved for LASIK. I ended up waiting until the end of year to make sure I saved up the funds to do it and had some time to heal. I also took advantage of an end of year deal they were offering. $1000 off was a pretty nice forcing function to get me to finally do it after prepping my eyes with the treatments. The procedure itself I couldn’t feel anything even though I was awake the entire time (medicated). Even the scary part when bubbles form on your eyes Dr. Benjamin let me know about in advance so when it happened I just went with the flow. I think the worst part was the recovery the few days after and coming home drugged up lol. I remember day 1 thinking omg what did I do to my eyes. My eyes just hurt and felt weird, but omg I could see 20/15 immediately. And then, day by day it gradually got better. It took a few weeks for my eyes to heal, but after 1 month you couldn’t even tell I had surgery. One eye was a bit more bloodshot than the other.I can see 20/15. It’s been life changing for the past year not having to worry about contacts or glasses. I just got married and was able to walk down the aisle with clear vision. I only wish I had the means to have done this earlier in life. If you’re considering it - do it! And definitely consider Dr. Benjamin to do it right.Lastly, to continue treating my dry eyes, Dr. Benjamin gave me an extra LipiFlow treatment, and also recommended I try punctal plugs (painless plugs inserted into your lids that help your tears last longer). He placed the plugs in my eyes under 5 mins. Wild.I heard from people during the LASIK process they have been nervous to get the procedure so I wrote this to help and because I think highly of this business for changing my life. And, some of the dry eye treatments I would’ve never known about had it not been for them. Again, I wish I would have had been able to do this years ago. It is so, so worth it!Thank you, Dr. Benjamin (and staff!)

Concerned about a reoccurring virus, I text Dr Benjamin’s office first thing in the morning seeking immediate advice and an appointment. In quick time I received both sage advice and a same day appointment. Dr B takes a personal interest in his patients well being and has diligent, competent and attentive staff to support the care and support needed. Can’t speak highly enough.

Been going to Dr B for nearly a decade after trying many other eye doctors in LA.  Beyond solving my chronic dry eye and being the most knowledgeable, he is an all-around great guy and spends time educating. The staff is wonderful as well.

Benjamin and his entire extraordinary team at the Benjamin Eye Institute.I was nervous as eyes are so sensitive,  but his incredibly kind receptionists and nurses make you feel at ease and diminish all fears right away . Dr Benjamin and Alba were great. He is very professional, kind and with clear explanations. I had an eyelid quick surgery and I’m just missing the glasses prescription, I left the office and forgot to ask for it. But he and his team were amazing!

I stare at computer screens for very long periods of time so I was nervous to make this life change but ready to ditch my glasses for good!
